T-Shirt Order Forms are now available on this website.
Individual Order Form Deadline: Sunday, April 30.
Group Order Form Deadline: Friday, May 5.

TEENS, PARENTS, ETC.:

Step 1: Download and print the Individual Order Form.

Step 2: Fill out the Individual Order Form, including how many shirts you want to order in each size (one form can be used to order more than one shirt).

IMPORTANT: If you want to order black shirts AND white shirts, you will need to fill out TWO separate order forms (one for black and one for white).

Step 3: Turn in your order form(s), along with payment, to your congregation's youth minister or youth leader no later than Sunday, April 30, 2006. (Make checks payable to your congregation.)

YOUTH MINISTERS / YOUTH LEADERS:

Step 1: Download the Individual Order Form and make copies to distribute to your teens.

Step 2: Teens fill out their order forms and return them to you by Sunday, April 30, along with their payment (checks should be made payable to your congregation).

Step 3: You combine all the individual orders onto the Group Order Form.

Step 4: Turn your teens' payments over to the elders or treasurer at your congregation, and get a church check for the total amount, made payable to "Bridgewood church of Christ".

Step 5: Mail the Group Order Form and payment to Jon McKenzie at the Bridgewood congregation (address is shown on the Group Order Form). Deadline for Group Order Form is Friday, May 5, 2006.
